Master Teacher Submission Spring 2021

In order to continue to allow teachers to upgrade their license or renew, the master teacher portfolio will be submitted electronically this year. Please completely fill out the form and upload all required documents. Because this year's submission is digital, it is critical that you observe all anonymity guidelines. Your name, students' names, school, and email should be redacted from all evidence.

Please be sure to attach the following in the "file attachments" area below. You can upload a maximum of 10 attachments: 1. Application 2. Recommendation forms - 2 must be submitted 3. Checklist 4. Narrative - Limit of 12 pages, double spaced 5. Evidence - Limited to 10 pieces. Evidence should be labeled and referenced within the narrative. Please upload all evidence as one file. (Tip: Use word to import pictures/screenshots of separate documents you may have)
